1. If you the PATIENT and the primary insured please bring your ID card .
If you don't have your ID Card you have to know your ID Nunber and your insurance company name.

2. If you are the patient by the insurance belongs to your relatives we need to know ID number and date of birth for the primary insured person.

3. If you have more than one insurance coverage we will be glad to file clames to both insurances. But we must to know wich is the primary insurance.
